Unlocking Your Benefits: Your KelseyCare Advantage ID Card
Your new ID card is your ticket to quality health care coverage for medical, dental, vision, hearing, and prescription drugs (if your plan includes Part D). Keep your red, white, and blue Medicare card, but discard any old health plan cards. Here’s how to get started:
Review: Make sure your information on the ID card is correct.
Present: Show your ID card to your doctor, dentist, or other provider at your next visit.
Call: You’ll receive a separate over-the-counter OTC card in the mail. Activate it by calling the number on the back to access your quarterly allowance for over-the-counter medications.

Empowering You: Appointing a Representative
One way to feel in control of your health plan is by designating an authorized representative through an Appointment of Representative (AOR) form.
Your Trusted Partner
When you use an AOR to appoint a representative, the person you choose can act on your behalf with KelseyCare Advantage. This could be a family member, friend, advocate, doctor, or anyone you trust.

Help with paperwork: Your appointed representative can assist with tasks like filing appeals or requesting reimbursements.
